The Electronic 1095 Consent Form

Employers are mandated by the IRS to have each employee electing to only receive an electronic 1095 (e1095) instead of a paper printed 1095 to fill out the “Electronic 1095 Consent Form”.

This consent form is documentation for the business in case a complaint is filed with the IRS.

e1095 Availability

The e1095 will be available on the secure website until October 15th of the issuing year. An employee can access the e1095 as many times needed until the October 15th date.
 

Note: An employee can receive a paper copy and an e1095 when filing with the complete option.  For an employee to receive both types of forms (electronic and printed), the e1095 Consent Form is not necessary.
