Michigan 5080 Form

The state of Michigan will accept multiple 5080 Forms. Processing this form through Aatrix we will be submitting only the withholding information. It will be the employer's responsibility to submit a second 5080 Form directly to the state of Michigan to report the sales and use tax information.
