New York NYS-45 Form Two State ID's

The NYS-45 is a Quarterly Combined Withholding, Wage Reporting, and Unemployment Insurance Return.

On the NYS-45 quarterly reports, there are two places for account numbers:

  • The first is the UI Employer Registration Number which is formatted as 7 digits, a space, and one digit, i.e 12345678. The final digit is a check digit, which is calculated by the numbers entered into the first field. This field is not editable. If you are unsure what your UI Employer Registration Number is, please contact the New York State Department of Labor.
  • The second account number is your Withholding Identification Number. This number is formatted as 9 digits, a space, 2 digits, a space, and 1 digit, i.e. 123456789 12 1. The first and second part of this ID is issued by the State of New York. The final digit is a check digit that is calculated with the information entered into the preceding two sections and cannot be edited.
